HOME HEALTH AIDES
When working with patients, insurers, or physician, DD Health Home Care believes that the optimal home care requires the use and availability of multi-disciplinary team of professional and competent Administrative staff.
Home Health Aide services:
- Personal care to our patients under de supervision of a Nurse or Licensed Therapist
- Assist with bathing
- Prepare, serve meals
- Light housekeeping
- Pick up groceries and medications for patients at local grocery stores or pharmacy
- Assist with ADL’s
- Assist with ambulation, transfers
- Assist with self administration of medications
Home Health Aide provide personal care to our patients under the supervision of a Registered Nurse or a Licensed Therapist.
CRITERIA FOR ADMISSION
Admission to this Agency can only be made under the directions of a physician, based upon the patient’s identified care needs, homebound status and the type of services required that we can provide directly or through coordination with other organizations.
